The business problem
What was going wrong
Incidents were recorded on paper forms that had to be found, transcribed and filed before anyone could act on them. By the time a report reached a manager the detail had degraded, and there was no reliable way to show that the required steps had been followed.
The solution
What we proposed
A mobile-first capture form that a staff member can complete on the floor in a couple of minutes, with structured fields that prompt for the information a review will need. Submission triggers notification and escalation rules, and every subsequent action is recorded against the incident, so the audit trail is a by-product of using the system rather than a separate task.
