Accord Incident Reporting

A mobile-first incident capture and escalation system, built so that a report filed on the floor becomes a reviewable record with an audit trail.

Details are sanitised — no client information is published.

Interface outline. Structure is shown; no live or client data is published.

The business problem

What was going wrong

Incidents were recorded on paper forms that had to be found, transcribed and filed before anyone could act on them. By the time a report reached a manager the detail had degraded, and there was no reliable way to show that the required steps had been followed.

The solution

What we proposed

A mobile-first capture form that a staff member can complete on the floor in a couple of minutes, with structured fields that prompt for the information a review will need. Submission triggers notification and escalation rules, and every subsequent action is recorded against the incident, so the audit trail is a by-product of using the system rather than a separate task.

Compliance evidence living in a filing cabinet?

We build systems where the audit trail is a by-product of doing the work.
